There is a reason and purpose behind disease. Sometimes illness can even be beneficial, for it can lead us into realms of understanding that were not open beforehand. Many will not accept the idea that illness is self-generated because we believe that no one would choose such an experience.

When energy is blocked from expression, however, when our own ideas and beliefs are at cross purposes with themselves an impasse is reached. The energy that is blocked manifests itself in the areas of the body that dutifully reflect an "inner" problem or challenge that is not being dealt with. The good news is that you can change the state of your health now and begin to live life again with exuberance and vitality. One day, I was coughing up a storm, but my cough was more aggressive and painful than I was ever used to. She told me that there was a website called WebMD that might be able to diagnose my symptoms, so I tried it out.

I clicked on the picture of the body and was taken to the symptom checker page which asked me first if I was male or female. Afterward, I answered questions about where I was experiencing pain. I clicked the chest area then answered a series of questions regarding the specifics of my chest pains. After my symptoms were determined by the checker, it was suggested that I call my doctor for an immediate appointment within 1 day. Since I had an appointment the next day, I went to find out that I had a sinus infection - not too serious, but enough to require antibiotics and prescription cough medicine for a month.

WebMD labels itself as "the leading source for trustworthy and timely health and medical news and information." And aside from having this handy symptom checking tool, they offer up-to-date information on a variety of medical topics, and even have what they call "health centers" that focus on various medical conditions from acne and asthma to sleep disorders and strokes. In addition, they offer information on the top 12 health topics, the most common treatments, diet assessments, necessary nutrients, drugs, skin care and so much more - in sum, this website has an overwhelming wealth of information. With everything they have available, it is no wonder that many believe it is one of the best health websites available.

KidsHealth

KidsHealth is another one of the many health websites available - and like WebMD it is very highly-ranked, said to be the most visited site regarding children's health. When you first enter the site, you are given three mini health websites to visit: the site created for adults, the one created for kids, and the one created for teenagers. I decided to start with the parents' website first to see what information they had available. On this site, they list a variety of links to choose from, depending on what you're interested in learning. Some of the links include infections, emotions and behavior, medical problems, and doctor and hospital information. Also, they give parents helpful tips in being able to positively care for their children as well as dealing with emotional and behavioral issues. In sum, this part of the site works to look at health from the parent's perspective to help them be a better health administrator to their child.

Of the three mini health websites, the kids' site offers the most kid-friendly information developed to help them learn with a little fun in tow. There are spelling bees related to health topics, recipes for health and fun foods, articles about growing up, understanding the body, and dealing with feelings, and even a glossary of medical terms written in very easy to understand definitions. Medical Billing - Multiple Batches

One of the most confusing parts of medical billing and the electronic submission of claims is the batch. Most billers don't understand why you even need to have multiple batches. Can't all the claims just go inside one package and get shipped? Well, with paper, yes. But if you're a big billing house and billing for a number of providers, then the process isn't that simple. Before we go into our detailed review of the YA0 record, an explanation of batches is probably in order.

Because of the way claims are paid, especially by the big insurance companies or government agencies, keeping track of batches is very important. Why? Because when insurance companies cut their checks for the claims, they are cut and paid to the provider, not to the patient, unless designated to do so. If a large billing house is representing multiple providers, they may very well submit a large claim file representing multiple providers. These providers are identified in the BA0 record in field number 2, positions 4 - 18, which is the provider ID. This record designates the beginning of that batch and this field designates the provider associated with that batch. If the biller wants to bill a second provider, then another BA0 record must be used so that the insurance carrier knows who the second carrier is. Otherwise, the check will end up getting cut for one provider. This can obviously cause a lot of problems.

In order to make sure this doesn't happen, most electronic billing software has what is called a multiple provider option. What happens is that somewhere in the system, a list of providers is kept in a large table. In very large billing houses, this table can contain hundreds of providers. Each one is given an ID number and a table number. The ID number is assigned by the governing agency. The table number is assigned by the software. Each patient in the system is assigned to a provider. When that patient's claim file is pulled, a cross match is made between the table number of the provider that is entered in the patient file and the provider number itself. If the provider number is different from the previous provider number that was billed, a new BA0 record is created. Before this is done, however, the YA0 record is created to sum up the previous batch for the first provider.

This is of course greatly simplified, but hopefully you get the general idea. A preferred provider option (PPO) is a plan where your insurance provider uses a network of doctors and specialists. Whoever provides your care will file the claim with your insurance provider, and you pay the co-pay.

Who am I allowed to visit?

Your provider will cover any visit to a doctor or specialist within their network. Any care you seek outside the network will not be covered. Unlike an HMO, you don't have to get your chosen doctor registered or approved by your PPO provider. To find out which doctors are in your network, simply ask your doctor's office or visit your insurance company's website.

Where Can I Get it?

Most providers offer it as an option in your plan. Your employees will have the option to get it when they sign their employment paperwork. They generally decide on their elections during the open enrollment period, because altering the plan after this time period won't be easy.

And Finally, What Does It Cover?

Any basic office visit, within the network that is, will be covered under the PPO insurance. There will be the standard co-pay, and dependent upon your particular plan, other types of care may be covered. The reimbursement for emergency room visits generally range from sixty to seventy percent of the total costs. And if it is necessary for you to be hospitalized, there could be a change in the reimbursement. Visits to specialists will be covered, but you will need a referral from your doctor, and the specialist must be within the network.

A PPO is an expensive, yet flexible option for your small business health insurance. It provides great coverage though, and you should inquire with your provider to find out how you can reduce the costs.

HMO (Health Maintenance Organization)

Health Maintenance Organizations (HMOs) are the most popular small business health insurance plans. Under an HMO plan you will have to register your primary care physician, as well as any referred specialists and physicians. And because HMOs are geographically driven, the options may be limited outside of a specific area.

Out-of-network visits will also require you to handle the paperwork, meaning submit the claim to the insurance provider.

EPO

The differences between an EPO and the other two organization plans are small, but important. An HSA is a tax-advantaged account used to pay existing and future medical expenses. HSAs are used in conjunction with high-deductible health plans (HDHP), which will make some with pre-existing conditions ineligible. Also, HSAs must be funded with cash. Communicating the terms of this account to your employees is important, as a large number of HSAs are underfunded or improperly funded. The health savings accounts were signed into the law by George Bush in 2003, and have become an affordable alternative to a group health plan.

When inquiring about an HSA, there will be a few things you will want to clarify. While HSAs generally cover routine medical expenses and copays, some can provide dental and vision care as well. And since HSAs can be combined with certain compatible plans, it is important to understand how money from the account will be allocated. And finally, you will want to know about cashing out your HSA balance. The amount is taxable and could be subject to a ten percent excise tax.

HRA (Health Reimbursement Arrangement)

An HRA is exactly what it sounds like. The employer reimburses the employee for health care. As an employer, you will usually have the option to contribute to a reimbursement fund, or to pay fees as they are incurred. These reimbursements can be deducted from your taxes, and are tax-free for your employees, saving you both money.

Some providers empower employers by giving them more options. HRAs, unlike HSAs, don't have to be funded with cash money, placing a book keeping entry on your balance sheet is enough. You can usually control aspects of your arrangement such as reimbursement limits, whether you or your employee pays first, and if the previous year's funds roll over.

HRAs are becoming a more popular option because of the control it has given small businesses. Combined with a high deductible health plan (HDHP), an HRA could be the most cost-effective solution to your small business health insurance problems. It's always best to compare these plans to PPOs, HMOs, and EPOs to know what works best.

Fee for Service (FFS) or Traditional Indemnity

A fee for service plan is the most flexible small business health insurance option. You choose your doctor, and your hospital. You can see a specialist without a referral. This flexibility, however, comes with more out-of-pocket expenses and higher insurance premiums.

The typical FFS plan has a deductible ranging anywhere from five to fifteen hundred dollars. After this amount is reached, the provider will pick up eighty percent of your medical bills, and require you to pay the remaining twenty percent. Because of the rising costs of health care, and the potential for a small number of doctor's visits to cost thousands, these plans can become incredibly expensive.

Flexible Spending Account (FSA)

A flexible spending account is a savings account to be used for medical expenses, and is funded by pre-tax dollars. You should be forewarned, money from FSA accounts cannot be rolled over. They are, however, available to use for two years and two and half months after the benefit year. A terminated employee won't be able to use leftover funds, unless there is a positive remaining balance and COBRA is elected.

With HRAs, FSAs, and HSAs, your employees can use debit cards for medical transactions. Be sure to research this thoroughly. You will want to be sure your debit card plan is IRS compliant, and that you can use a large number of pharmacies. You should also pick a plan that can verify eligibility on the spot.
